PreMarital Mentoring Forms

Thank you for your interest in premarital mentoring at Haven of Hope Ministries. We require that you complete the paperwork below and the Taylor Johnson Temperament Analysis tests before we schedule your sessions. Once we receive your completed paperwork, I will order your TJTA tests.

Due to the intensity of the sessions, we also ask that you take time off work for this time of mentoring.  It is important for you to have plenty of private time away from normal duties and obligations.  We ask that you do not bring children to the sessions.

Please pray for wisdom and direction during your mentoring.  We realize that it is not about us as mentors, but it is about our Savior and Friend, Jesus.  If you have any questions, please call the office at 616-693-2925.  I look forward to receiving your completed paperwork.
